Medical Forms

• Class 1 and 2 Personal Health and Medical Record (PDF)- To be filled out by Parents before Scouts can attend certain troop events.

• Class 3 Medical Form (PDF) - To be filled out by Scouts and Parents before they  can participate in high adventure activities, athletic competition, and world jamborees.  Annually, to be used by adults over 40 for all activities requiring a physical exam and applies to all Wood Badge participants/staff regardless of age.

• Parental Permission/Release/Medical Treatment Form for all Troop Events (WORD) - Parental Permission for Scouts to attend certain troop events.
  

Merit Badge Counselor Application Form (PDF) - Do you have a special area of expertise through vocation or avocation? There is probably a BSA Merit Badge for that topic!  You can help Scouts advance by volunteering your services as a merit badge counselor.
